2. ENTRY. This Sweepstakes begins at 12:01 a.m. EST on November 5, 2002 and ends at 11:59 p.m. EST on November 16, 2002. Two ways to enter: (1) complete and submit the online survey, or (2) Print your name, address (no P.O. Boxes please), business, business address, email address and daytime phone on 3.5" X 5" index card and mail to www.SurveyPro.com Online Store Sweepstakes, 1361 N. Lakeview Dr., Provo, UT 84604. Limit one entry per person or e-mail address regardless of method of entry. All mail-in entries must be postmarked by November 16, 2002 and received by November 25, 2002. Online entries must be received by 11:50 p.m. EST on November 16, 2002. In case of dispute as to identity of entrant, entry will be declared made by the authorized account holder of the email address submitted at time of entry. "Authorized Account Holder" is defined as the natural person who is assigned an e-mail address by an Internet access provider, online service provider, or other organization (e.g., business, educational, institution, etc.) responsible for assigning e-mail addresses or the domain associated with the submitted e-mail address. Any other attempted form of entry is prohibited; no automatic, programmed, robotic or similar means of entry are permitted. Odds of winning depend on number of eligible entries received. 3. DRAWING. A total of one (1) Grand Prize Winners and five (5) Second Prize Winners will be selected in a random drawing to be conducted on or about December, 2, 2002 from all eligible entries received. Winners will be drawn by an independent judging organization, whose decisions shall be final and binding. Winners will be notified by phone or email and confirmed by mail. If selected winners cannot be reached within 24 hours of first attempt to notify such winners, alternate winners may be selected.

5. ELIGIBILTY. Employees/Agents (and their respective family members) of Sponsor and its parent company, affiliates, officers, directors, agents, employees, subsidiaries or service agencies [or independent contractors] are ineligible to participate or win a prize. Winners may be required to sign (within 24 hours of telephone notification) an Affidavit of Eligibility, Compliance and Release Form ("AECR Form") certifying eligibility and compliance with these rules. By entering, winners agree to hold Sponsor harmless from any and all liability arising out of participation in sweepstakes, the acceptance, use or misuse of prize and authorize the use of their name, photograph, and likeness for advertising and publicity without further compensation, except where prohibited by law. Non-compliance with these rules will result in disqualification and forfeiture of prize. An alternate winner will be selected by a random drawing. By participating, entrants agree to be bound by the decisions of the judges and the official rules.

6. CONDITIONS. Once submitted, entries are the sole property of Sponsor, and will not be acknowledged or returned. Winners agree to hold harmless and indemnify Sponsor, its parent, subsidiaries, affiliates, directors, officers, employees and agents against any and all liability, damages or causes of action (however named or described), with respect to or arising out of entrant's participation in the promotion or the receipt or use or misuse of the prizes awarded herein. These parties shall have no liability or responsibility whatsoever for lost, late, misdirected, destroyed, damaged, incomplete, illegible, postage due mail entries, or other errors of any kind or for technical, hardware, software, telephone or other malfunctions, errors or failures of any kind, lost or unavailable network connections, Website, Internet, or ISP availability, unauthorized human intervention, traffic congestion, incomplete or inaccurate capture of entry information (regardless of cause) or failed, incomplete, garbled, jumbled or delayed computer transmissions which may limit one's ability to enter the sweepstakes.. Sponsor, its parent, affiliates and subsidiaries, and each of their directors, officers, employees, agents, successors and assigns have no liability or responsibility for any personal injuries, losses or damages or any kind resulting from participation in the Sweepstakes, acceptance, possession or use/misuse of any prize.

7. ADDITIONAL TERMS. Sponsor reserves the right, in its sole discretion, to cancel, terminate, modify, or suspend this sweepstakes should (in its sole discretion) virus, bugs, non-authorized human intervention or other causes beyond its control corrupt or affect the administration, security, fairness or proper conduct of the sweepstakes. In such case, Sponsor will select the winner from all eligible entries received prior to and/or after (if appropriate) the action taken by Sponsor or from mail-in entries only. Sponsor reserves the right, at its sole discretion, to disqualify any individual it finds, in its sole discretion, to be tampering with the entry process or the operation of the Sweepstakes or website.
